What each field protects

Action item + owner
Use a concrete verb and name the person responsible; do not infer an owner when none was agreed.
Due date + status
Use an explicit date when available and update status only when later evidence supports the change.
Source quote + source
Preserve the exact supporting words and identify the recording or transcript where they appeared.
Last reviewed + notes
Record when the item was checked and what must be confirmed in the next conversation.
